Question regarding FBI PCC application

Hello,

On https://www.fbi.gov/about-us/cjis/identity-history-summary-checks/submitting-an-identity-history-summary-request-to-the-fbi, in step 4, there is a 'Identity History Summary Request Checklist' link.

When we click on that link a pdf document opens and in that document, the last item is 'Include a form of contact information (i.e., e-mail, telephone number) in case we need to contact you.'

But I don't see a 'form of contact information' anywhere in the website. Can anyone who has obtained the FBI PCC clarify?

I submitted Applicant Information Form, Credit Card Payment Form, and Finger Print Card.

Contact information is part of Applicant Information Form.

Yeah, sorry. I thought they were asking us to fill out another form.
