Hello,

My Japanese spouse recently received an email from the visa office in Manila regarding a file being created for her. My belief is now her application is in process. Along with the included email is another form, Appendix A, Passpory Request-IRPA. She is supposed to send this in within the next 45 days.

We are very confused. In the form it asks for her to fill in her details and any accompanying and non accompanying family members.

Do we fill in her details as the principal applicant and my details in the spouse section? We have no children so that's blank. Furthermore, do i need to fill in a copy too? I'm the Canadian and she is my common law spouse.

Finally, does she have to submit her passport with this form? Do I have to as well??

We are really flustered and confused :(

I hope somebody can help us out. Thanks.

Yes, "she" as principal applicant fills it out, "yes" with you in the 'spouse section'.
No, you don't need to fill in/submit one also.
I would normally say "yes" but I don't believe Japanese must send their original passport, (read the letter carefully). No, you don't need to send your passport, nor a copy of it.

My husband and I just recently received the same request letter. For the Appendix A, we were told to fill out my husband (Applicant) and my information, which included the following; passport #, passport expiration, height and eye colour. We have no kids so we left those blank. As for passport, my husband is from a visa-exempt country (Korea) so the letter dind't indicate anything in regards to the passport.

Unless my English is poor, it's safe to say she doesn't need to include an original of her passport eh??
 
We didn't send a passport because they didn't request us to. Your letter is almost identical to ours, except they asked us for additional documents :( and that is the correct address.
 
I received the same document from CIC Manila in late August. I was confused so I emailed them back for clarification. They told me

1. My South Korean wife (principal applicant) and I (sponsor) need to be included in Appendix A. We also have no kids.

2. We don't need to send her passport.

3. We could complete Appendix A, she could sign it, scan it and return it by reply email. No need to use snail mail.

I'm confused, isn't Appendix A just the document checklist? There is nothing to "fill out", just tick off documents that are included in the application, no? And what do they mean here by every non-accompanying relative "filling out" this form as well....if they're not accompanying there will be no need for them to furnish the listed documents, no? Or are we expected to send a "blank" App A for each relative listed on IMM5669 (I believe it is). Seems like many applicants are getting this notification, and I'm feeling like I missed something somewhere. (Also didn't see any place that asked for a signature on this document.)

I think it is more likely they mean "schedule A" which is the background declaration that all applicants must file. This is the one that all members of the application must complete and sign whether they are accompanying or not. Which usually includes, primary applicant and anyone considered a dependent to primary applicant (unless said dependent is a canadian citizen already). If the passport is requested and you are from a visa exempt country, then you can usually just photocopy the passport (in it's entirety) and then send that along with the requested document. Usually you can email this information to the VO, however I would also suggest putting a note in this email that you have also posted the original documents to them and they should be received in "x" amount of time. This will ensure you have met the 45 day deadline and also allow them to usually continue on the processing of the application while they wait for the 'hard copies' to be received.

You should call your VO to ensure they mean schedule a and not appendix a. If they do in fact want schedule a here is a link for you to get it pdf format. http://www.cic.gc.ca/english/pdf/kits/forms/imm5669E.pdf

VO= visa office. So whomever sent you that email is whom you need to contact. Where is your visa office? You should be able to locate an email for them or a contact phone number on the CIC website.
 
So "Schedule A" is effectively IMM5669. It would make sense that the VO would ask for this if it wasn't included in the application package as it is a core form to the application -- and it is "filled out" and "signed" (unlike "Appendix A" which requires only "ticking" items off & no signature). I have seen a few mentions elsewhere on the Forum of people getting the same request from their VO, i.e. to send in a completed "Appendix A". Echao, can you verify that they have requested SCHEDULE A (also referred to as IMM5669) & not Appendix A? Thanks!

It is not the same as IMM5669 or the document checklist. I believe the form is specific to the Manila office, as I've not seen any other VO send it out.
