Hello, im applying under Spouse with dependent. My question is that do i still need to fill out the IMM 5604 for my son who is 12yrs old if on he's birth certificate only shows me his mother as his sole parent? Can they go just by this information? I googled around and talks about getting an affidavit signed and notarized. Is that enough? And do i still print the form IMM 5604 with out filling it out and put the notary on top of this? Thanks so much.
