Keep it blank, they will issue you New uic... moreover there is no option to mention that your student visa was refused.

I read that, but it is only for "refused refugee, pr, pnp, csq, temporary", not for study.

Moreover, there is no uci code for study visa. They only issue 10 digit file number.

In Canada, there is no study visa such thing, to study in Canada you need study permit(to grant your status)+temporary residence visa(to be able to enter Canada)+CAQ(for study in quebec only)
here you can see study permit is under the category "temporary residence visa" : http://www.cic.gc.ca/english/information/times/temp.asp
And for your uci, you can find it on your study permit if you have one, it's printed as client ID(see more here:http://www.cic.gc.ca/english/helpcentre/answer.asp?q=013&t=4)

so if anyone got their study permit refused before, i'm afraid that he has to mention it in Schedule A, question 6 as Lammawitch said.

Sir, I mean to say that in form 5669E question 6 (d) they haven't mention the study permit but you guys said study permit comes under TRV so I was trying to explain that there is difference between Study permit and TRV. So we need not to mark yes.

So my girlfriend's application got returned as incomplete :( It has a checklist that just says the declaration of intent to reside in Quebec wasn't signed (name was written on computer and printed out, we forgot to sign it in handwriting) and the original docs include marks that they've made. My question is... can we reprint the declaration of intent to reside in Quebec, sign it properly and resend the exact same documents that were sent back to us and marked? We also have 2 envelopes, one of which has "not necessary" marked on it, I think it has our passport pictures in it. Is there anything else we should do?

Thanks a bunch for your help, really stressful times :(
