Proof of Medical test

Hello everyone,

I took the medical test recently and received the below sheet + the receipt of payment. Is that how the sheet is supposed to look like? I've read that others received 2 papers, one of which is some consent?

It looks like this yes! Personally I didnt sign any consent form!! I just uploaded the proof and that was all..Meds passed no problem!!

Raise CSE and send them a scanned copy of your IELTS report form.

I did my medicals using old passport now iam going to change to renew and get issued a new passport .should i do do medicals again .I did medicals 3 weeks ago and hospital refusing to do again but i need to change my passport because there r ink stains in old one

hi! you should check the back part of that page because it is printed back to back. well that is mine looks like. it has 1 page but has a print back to back. at the 2nd page you will see the "consent" hope that helps.

What? I didnt even look at the back but I think its blank.

I scanned only front page and applied. Hope I didnt make a mistake. But Im sure its blank behind.

oh im not sure. because mine is the same way the pic is in front then at the back there is a print and i included that in my application.

My dr sent me a pdf file and i just uploaded it but no consent

On my medical exam pdf file it is written the following;consent provided ;then its written on date x you consented online to using emedical to process your health examination where available

I have the exact one, it says PAGE 1/1 so nothing on the back.

I have another paper where i had hepatitis a,b and c tests not sure if they are required for all or only for health professions

I did sign a consent form at the clinic, but they never gave it to me.
