Proof of Medical test

I have another paper where i had hepatitis a,b and c tests not sure if they are required for all or only for health professions

I think they ask for hepatitis tests for those who might have a risk of catching it. If you're a health professional, I'm guessing you're at risk, hence the test.

I did sign a consent form at the clinic, but they never gave it to me.

That's correct. You just need to sign that consent form so that the clinic can release the report to CIC. We do not need to upload it. Even I did not get that form.
