Proof of Medical Exam

Hi,

I am confused about the proof of medical exam that we need to upload.

On the website, it says that the medical clinic will give me the form IMM 1017B for uploading. But when I did my medical exam, they just gave me the e-medical form with my photo, a UMI bar code and the stamp of the clinic where I got the medical exam done from. It has my personal details and the list of medical exams performed.

Is this okay to upload? Am I missing something?

You just need to upload the form with the UMI number. they extract the report using that unique number.
