Proof of Citizenship - Document Question.

Hello, I'm applying for my children's proof of Canadian citizenship. They are currently American born citizens (I was born in canada).

One of the document requirements is "Primary Identity Document" which states:

Upload a colour digital or colour scanned copy of the front and back of a government-issued ID, such as:
hospital card
school records
health insurance card
age of majority card
certificate of Indian status
senior citizen's card


My question is: their American insurance cards do not have DOB on them, they are homeschooled so no school records. We don't have hospital cards here besides the one where they have the babies footprint on them. I'm not sure what to use?

Please help!

I applied for my kid a few years ago. Let me see if I can find the comprehensive list of docs I submitted. The kid was few months old and only had these two ID’s at that time other than the birth certificate.

List of documents submitted

• Completed and Signed Application (CIT 0001)

• 2 Photos of kid (Minor Applicant)

• 2 Identification documents for minor applicant: Passport Photo page and Passport card

• Proof of Payment

• Birth Certificate for minor applicant

• Citizenship proof of Canadian Parent

• Canadian Passport of Mother
