Problem with online application of off campus work permit

Hello,

I am applying my off campus work permit online, and it requires me to upload the proof of medical exam,
either IMM 1017B Upfront Medical Report form or the information printout sheet.
But I have already done the medical exam in Hong Kong before I came to Canada when applying study permit, and the result was sent by the medical institution directly to CIC but not me, so I have no IMM 1017B Upfront Medical Report form or the information printout sheet.

Can anyone tell me what I could do?

Write a word document explaining this, I have the same problem and i called CIC and they said to upload a word document stating the reason for not submitting it, don't worry it does not really make a difference.

I'm in the process of applying my OCWP as well. I'd been wondering about that form - thanks for the information! Do you know if I need a job offer letter before applying for my permit? Or do I look for a job after i have received my OCWP?

on your on campus job .. how much you earn...just give me an idea .... and after doin off campus job how much can you save in 1year(save means that by earning those money can you fulfill your tuition fees+home+food+everything ),,,
