Problem with Birth certificate

Hi,

I have a Birth certificate without my name on it.. I am looking for other options. Please tell me what would be better way.

1. BC without name + Affidavit from my father from india + my 10th class certificate.
2. Affidavit from me + my 10th class certificate (with out birth certificate).

3. Apart from my above question, Do i need to notarize 10th class certificate? can this be done from india too? because i have my certificates in india.
4. My father's Last name is in initials only on my 10th class certificate, will that be a problem?

Please suggest me.

As the agitation going in my state i cannot add my name my birth certificate. Its been 2 months i have been waiting for agitation to end.

Satish ! You can have your 10th class certificate notarized in Canada and submit it along with a covering letter for your father's surname. If the notary is not done without looking at the originals, get notarized from from any authorized notary in India and submit along with a covering letter explaining that your father's surname has just initial. I suggest you not to add your Birth Certificate at all.

Do i still need an affidavit from my self ( I can have one from canada) & notary for my 10th class certificate from india.

From other posts i think either birth certificate or secondary school certificate or affidavit seems to be sufficient for proof of birth.

My secondary school certificate has my name , DOB & my father name. Can i send it alone or affidavit also needed.

As many cases they are not considering proof of birth from consulate i think i better not to take it.

Is there any person who has sent secondary school certificate & got returned/rejected application?

Dude.. if your 10th certificate has your name DOB and father's name, that would be sufficient. AFAIK, my friends have submitted like this and it was not returned.

I was not having BC and my 10th certificate had only initials of my Father and mine.

I included below supporting docs and got MR.

10th certificate.
Certificate of dob from indian consulate.
Affidavit done back home by mother.
Affidavit done by me here in canada.
And an explanation letter.
