PROBLEM:IMM 5710 not Validated because of no data applicable

To whom this may concern,
I encoutered a problem on validating my IMM 5710 form (for Work permit extension)

COMING TO CANADA
3. Date and place of your most recent entry to Canada Date Place
(if not the same as original entry) yyyy-mm dd

I just entered Canada once in Toronto International Airport (June 2,2011)

Fist, I left the date blank, then put Not applicable on place but It was not validated
second, I entered my date of original entry, not validated again.

What can I do with this problem?

Hi!
Just download this form again and delete previous one. Its can be problem with downloaded form. I met such kind issue too.

Leave it blank if there are no choices of NOT APPLICABLE.
