I became Permanent Resident of Canada first time on September 17, 2005.
My PR Card expired on April 29, 2020
I am living outside of Canada in Pakistan since December 30, 2019

Which dates should I fill in the above Column?
If you have not returned to Canada at all in the last five years, then your absence is complete and you need to list the entire period as absent from Canada.

Note: without having convincing H&C reasons for your absence, or without being able to count your time abroad (e.g. for accompanying a Canadian citizen), your application will almost certainly be denied, and you will likely lose your PR status.
 
You need go apply for PRTD based on H&C unless you have a Canadian citizen spouse who has been living with your in Pakistan during this time period. We’d still need more information if this is the case.


In terms of the dates you are supposed to use you would use the 5 years before today’s date.
