PR Card and name change

Hi,
My wife's permanent resident card expired in 2010. She now wants to apply for a new card. However, we got married in a couple of years ago so she wants to change her name on her next card too. What form should she use to apply for a new card and have a name change on it? Does she just need to send a photocopy of her passport and marriage certificate along with a regular application (IMM5445)?

She should change her name before submitting application for new card. It can be done at the Service Canada office or according to the province where she is.
