I am writing a cover letter to request police certificate.
In the form, there is a section: "please mail the results of the check to the following address:"
Should I provide my address or visa office's address?

Assuming you have not sent your full application yet. I would suggest to get it delivered to your own address and send it along with your full application to your local visa office.
