From lookinh into this forum and one another forum most upfront applicants are getting delayed.
If u hadnt do upfront u would have received mr in january start or decmber end.

Sept or Oct applicants did upfront med , and their meds are already expired or will expire after 90 days , they need "medical reassessment" before PPR that's why their files are getting delayed

Extension or Request Medical Exam again

You are right . I did my upfront last March , I am sept 20 inland applicant, so haven’t got ppr or bgip coz of my medical , hope will shortly get ppr once medical will be re assessed .

I was searching for the medical exam extension policy of IRCC and found the following in their website

"Effective June 1st, 2012, officers can request that an applicant’s medical file be reassessed by a Medical Officer (MOF) or a Health Branch (HB) delegated staff for the issuance of a new medical certificate. The process applies to cases where a final decision of an officer is pending only due to the expiration of a medical certificate or when the validity of the medical certificate is about to expire and the applicant is unable to land before it expires. This process is limited to applicants who had a routine immigration medical examination (IME) done by a panel physician within the last 15 months in or outside Canada. The new medical certificate issued will be valid for 12 months."

I don't know but I read from old posts that most inland applicants get extension

Medical Reassessment process usually takes a week to a month ( i am not sure if this true )
