Seniors,

I am collecting my documents for full application to CHC SINGAPORE.

I am INDIAN National, I chnaged my last name from "Kumar" to "Sharma" while applying for passport in 2005, all my educational certificates have my last name as Kumar. Should i provide some written explanation for this or an affidavit saying i changed my name etc.

I have an open thread for Job responsibilities from current employer/ reason for not providing reference letter from current employer, Please provide your advise on it.

For the partial change of name, a written explanation [with some supporting docs] would suffice. However, if u can attach an affidavit, much the better.
