Are we saying to print/sign on each and every page of the physical presence calculator, rather than just the last page where it asks for signature and date?
As for printed name, my document is just called "Calculation 1" because it didn't say it should be named after myself etc.
As far as I am aware, all I need to do is print it off, all 17 pages in my case and sign and date the last page and that's it, the fact that it is included with my application is enough to identify it to myself.
