I currently hold a PGWP which expires in June 2018. I am eligible for a PGWP that is valid until June 2020 but was issued one that expires with my passport.
I have renewed my passport now and have my paper application for PGWP extension ready.
I have 2 questions regarding this,
First is which condition do I check in section 1 on the form 5710.
The second question is about the application for a Visa (sticker on passport). How do I go about this? To re-enter Canada by air from any country I will need one. But my current PGWP application does not need me to send my passport with the paper application.

2. Restoration of status as a worker
3. An initial work permit or permit with a new employer
4. Temporary resident permit.
