Personal particular form for PCC from Indian consulate Chicago

Hi I have a question about the Personal Particular form mentioned in this link http://indianconsulate.com/page/display/86/17. It is mentioned to download and fill the application form, Personal particular form, but the fields in the personal particular form don't seem relevant. Do we need to send this form along with other documents? If so what should be filled in 8(a) - present address(I am currently residing in USA) and in 10 - References - should it contain contact information of persons residing at present address or permanent address. Any help will be greatly appreciated. Thank you

Yes you need to fill it. Present address is US address. References are for India address in case your case is referred to India.
