if i (sponsor) pay the spouse sponsorship application fees online through my spouse's email, do i need to include Use of a Representative form? I am just thinking because on the official payment receipt it mentions the time of payment with time zone which is Eastern Daylight Time (EDT)

no you don't have to do use of representative form that's is only proof you paid for application, any one can pay for application.
