Passports Submission & Updated Information Chart

Hi,

Finally after 4 years I received passport request yesterday.

I have some questions about "Updated Information Chart";

1- The "Updated Information Chart" is not fillable form, so is it okay to print the form and fill it by handwriting?

2- In our passports there are no "First Name" or "Surname", we have the Full Name in one line as below;

"Name: First Name + Father's Name + Grandfather's Name + Family Name"

So which part should I fill as "First Name" and which part should be the "Surname"?

- One more question about send passports back after stamping, (we are sending our passports from Saudi Arabia to London visa office):
Can I included self addressed pre-paid return envelope (courier envelope), and ask visa office to use it when return passports back?
