In my case it was just the passport, Print of the PPR email and a self addressed return envelop.
Some people on the forum had been requsted 2 photos and Biodata form. It depends case to case and officer to officer.

The losing passport thread made me super paranoid. Now I am doubting everything including the spelling of the address etc.....
