The letter that I received from Mississauga state: "It is important to note that you MUST NOT complete the Visa Office Specific Instruction posted on the website. You must use the attached document checklist"

Rajinder you need to follow the checklist that Mississauga attached to your letter.


They (Mississauga) recommended :
1. Your parents take the steps now to obtain their police certificates from each country to avoid future delays.
2. You pay and submit proof of payment of the right of Permanent Residence Fee with the complete application.

Hi Experts,
parents need to fill supplementary information form Delhi & Medical appointment form.
if parents`s DOB 1941 & 1942 & married 1963,how can they produce bith certificte & marriage certificate.Is not passpot fulfilled all requirement becoz on passport mentioned everything.Do they need to produce additional documents for birth & marriage or passport is enough.plz i need prompt reply

If you cannot produce birth and marriage certificates, its better to include a letter (affidavit) stating the same and ask them to consider the info in the passport. CIC call center told me the same when I ask them about this. And that is what I included in my application.
 
@ Shuga: Congratulation and thanks for the update.
Now this starts making me nervous. I have sent documents few days before your and there is no change in my status. I have called CIC call center today and there is nothing in their system that they have received those documents. I understand that my case is different as my parents file is already in local office and medical is done but still hoped that they would start working those docs already. I can`t even think or worry anymore.
 
All of you guys are lucky getting those letters from Immigration. I have submitted my application just recently in middle of January.. SO I GOT LONG WAITING TIME I THINK..... BUT ONE THING I DON'T UNDERSTAND IS MY ORIGINAL APPLICATION WAS SENT IN DECEMBER 2007, NOW I AM BEHIND THOSE GUYS WHO SUBMITTED THEIR APPLICATION AFTER ME..... Hi guys,
I received a letter from CIC today stating that one doc IMM 5406-Additional Family Information is missing for my parents. They have returned my complete application and asked me to resubmit with the completed docs. But I had sent them these forms fully completed for both my mother and father Wonder if some idiot agent didn't see my application properly...very annoyed! Not sure what to do. Should I send them a cover letter stating that these forms were already submitted and resubmit the complete application?
Also, when filling Part 1 of this form, IMM 5406, for my mother, who is the principal applicant, I put her name and then my father's name as the spouse. Now when filling the same form for my father, who is the spouse, should I leave the Principal Applicant column empty so that it does not cause confusion? May be that's why they sent it back as I ahd written my mother's name as principal applicant in both and then filled the rest of the sections separately for both? So not happy with this...Any thoughts/ help would be appreciated.

Hmmm. its annoying and disappointing... Probably they completely misunderstood your application. This what I did in my application. My dad is principle applicant, my mom is spouse.
1. Completed generic application (IMM0008) for my dad and included my mom as dependant
2. Completed schedule A (IMM5669) for both my Dad and Mom. My Dad's application says he is principle applicant and my mom's application says spouse
3. Completed additional family (IMM5406) for both dad and mom. My dad's application says he is applicant and my mom as spouse. My mom's application says she is applicant and my dad as spouse.

I guess in your case they may be confused... I suggest you to talk to CIC call center and go from there. They might ask you to include a detailed cover letter explaining all these.

"in my father's form, my father as the applicant and my mother as spouse and in my mother's form, my mother as the applicant and my father as spouse?" This is how exactly I filled in additional family form.
Lets wait for the input from other people in this forum as I haven't received any information from CIC after submitting, to say concretely this is how they wanted (as per the guide it seems to be). Or you can call CIC to confirm how they wanted it.

Hi tricky,

i left the Applicant Name field blank in the IMM 5406 form for my father who is the spouse of the principal applicant (principal applicant is my mother)

I can't call CIC as I am out of the country for a couple of months :(
I doubt though that my application form was returned due to this minor error....I still feel that the agent just didn't see the package carefully. Anyway, if someone else has other thoughts/experiences, please share.

I was wondering about 5604 too. Like you said that you left the applicant information blank, so how can we fill the "section c" cause we have to fill information regarding brothers and sisters of the applicant. And in your case, you filled only spouse information....
Just confused with this whole 5604 logic....

@Browneagle That was a very valid point. 5604 has to be filled (declared) by principal applicant and spouse, one copy each. In this case, in fathers copy, father (being applicant) has to declare his sons, bros & sisters and in mother's form mother (being applicant) has to declare his sons, bros & sisters. Both has to sign on their corresponding copy. That is the main reason why I had it declared like that.
 
My understanding on this 5604 form is like...

(1) You need to fill separate forms for each applicant who is co-applicant...
(2) Your mother and father will have the same name for daughter and son, but their father, mother, brother, and sisters are different....

So yeah, You need to fill out the separate forms for each of the member....
