You can get affidavit from Notary Public. if CIC accept then CIC will send some document.. applicant to sign

In order to continue the processing of your application, we request that you read the following and return the signed acknowledgement on this letter within 30 days.

All family members, whether accompanying or not, are required to be examined at the same time as you. The only exceptions are a spouse from whom you are legally separated but not divorced, or if you have children for whom you do not have legal custody. These persons are not required to undergo medical and background screening in connection with your immigration application. However, if they do not undergo this screening, you cannot later sponsor them as members of the Family Class as they will be described under section 117(9)(d) of the Immigration and Refugee Protection Regulations which stipulates the following:

117(9) A foreign national shall not be considered a member of the family class by virtue of their relationship to a sponsor if:

(d) subject to subsection (10), the sponsor previously made an application for permanent residence and became a permanent resident and, at the time of that application, the foreign national was a non-accompanying family member of the sponsor and was not examined.

The decision on whether or not to have a non-accompanying family member examined is your decision. If your non-accompanying family member is examined, you may choose to sponsor them as your dependent at a later date. However, if you choose to not have your non-accompanying family member examined, the non-accompanying family will be excluded as a member of the family class. If you sponsor a non-examined non-accompanying dependent, and an officer determines that the previously non-accompanying dependent could have been examined during your own application but you chose not to make the applicant available for examination or the applicant did not appear for examination, the application will be refused pursuant to R117(9)(d).

Please complete and return the Non-Accompanying Dependent Acknowledgement within 30 days.

Thank you for your attention to this matter.

Please check one of the following:

____ I do not wish to have my dependent xxxxxxxxxxxxx examined for immigration purposes. I acknowledge that, since the above dependent is not being examined, I may not in the future sponsor him/her as long as R117 (9) (d) of the Immigration and Refugee Protection Regulations remains in force.

OR

No personal nor business travels? The reason I asked is because read newspaper that a person got denied due his failure to disclose his business trips.

Dear

Thank you for choosing Canada! We know this is a very exciting time in planning an important change in your life.

We have received your application for permanent residence. We expect to begin working on your application very soon. We will need time to review and assess your file. We should be ready to communicate with you again shortly. We understand that the waiting period can be difficult but please be rest assured that we have not forgotten about you.

We appreciate the time you have taken to prepare your application.

A couple of important things to know:

Your application number: xxx

This is your file number, and it is specifically for this application and anything to do with this application - like documents we ask you to submit, or medical exams we ask you to undergo in connected to this file number. It’s yours and yours alone.

Please use this application number for any communication with us about this file.

Your Unique Client Identifier (UCI): xxx

This is your unique identification number, it’s all about YOU and every application you make is connected to your UCI. It is as unique as you are. You will keep this same UCI for all the subsequent applications you may submit to our department in the future.

We want to process your application in the most efficient way possible and need your valuable help.

Here’s how you can help us:

· Create a MyCIC account and upload any documents that we request through your account. Documents submitted this way reach an officer faster. To create a MyCIC account, please visit our website: http://www.cic.gc.ca/english/e-services/account.asp.

· contact us if there is a change in

o your mailing address, e-mail address, or phone number

o your marital status, or the marital status of your dependents

o your family composition, such as a birth or death in your family

o your passport (you have obtained a new passport for you or your dependents)

o other information relevant to your application (for example you want to withdraw your application, or you want to add /revoke a representative)

· send information once, using one method only (MyCIC or the Web form or the IRCC website or e-mail or mail). We know you want to be sure we receive it, but sending us the same information multiple times pulls resources away from file processing.

· send all attachments ( for example: birth certificate of a new child, new passport information) in PDF format for faster processing.

· be patient. We don’t want to miss any of the information you have provided. Your application is in queue for processing and we will contact you if we need any additional information.

What else can you do to make sure your application is ready for processing?

· If you have not already done your medical examination, please ensure to do it as soon as you receive the medical instructions and forms.

· If your sponsor has not already paid the Right of Permanent Residence Fee (RPRF) please see our website for how to pay fees online: http://www.cic.gc.ca/english/information/fees/index.asp. RPRF has to be paid for you and your spouse only.

· If you have not already done so, obtain any required police certificates for you (and any dependents 18 or older). Please see our website for how to obtain police certificates: http://www.cic.gc.ca/english/information/security/index.asp.

PLEASE DO SEND THE FOLLOWING DOCUMENTS: confirmation of medical exam form, copies of RPRF receipts, police certificates for you and your accompanying family members.

Following the instructions listed above will help us use our office resources to process your application more efficiently.

In anticipation of our next communication with you, take a look at what awaits you in Canada and learn more about Canada, at http://www.cic.gc.ca/english/newcomers/live/index.asp.

Topics include:

· Get to know Canada

· Improve your English and French language ability

· Living in Canada

You are an important client for our office and we appreciate your patience and cooperation in helping us serve you better.

Sincerely,

The New Delhi Immigration Section

Received a request this morning for Passport Photos, not for Principal Applicant but for Spouse.

Dear XXXXXXXXXXXXX :
This is in reference to your application for permanent residence in Canada. In order for us to continue the processing of your application, we require the following documents:
 Passport photos: Passport size photographs (photographs must conform to the specifications outlined in the Guide to Immigrating IMM 3999 and Appendix B Photo Specifications) for your spouse only. This must be received at this office by: 2018/08/13

As we are from UK and there is no Visa required for UK Citizens to visit Canada we did not expect a Passport Request so hopefully, we are now nearly at the end.

Hi autocat,
I received the same letter this morning. Did you also receive any medical request form with this letter? (or will this letter be sufficient to show to the doctor?). There were no attachments on the letter or myCIC account.

Was confused if they are requesting the medical, pcc, and rprf receipt or just outlining procedure?

Hi Autocat,
Can you simply confirm if you received the IMM 1017 form attached along with this mail or in your myCIC account?
Thanks

If you're being asked for travel history, just mention that he never traveled outside the country. No need to provide detailed travel history within the country.

if he/she visited other countries, not within the country. Where, when, and why(holiday, business, etc)
