Parent Sponsorship - IMM 5669

I'm applying PR for my dad as parent sponsorship. My mom has the PR card. Just wondering do I have to fill out IMM 5669 and IMM 5406 for my mom as well?

Why not have your mom sponsor your dad? At least undertaking will be shortened.

Is your mom in canada now? Are they still married? Does she meet her RO?
If she is in Canada, meet her RO and they are still married, then ask your mom to sponsor your dad instead.

She has the PR card, but she doesn't live in Canada. However, she comes to Canada live every 2 years out of 5 in order to meet RO. She's married with my dad.
So back to the question, do I need to fill out IMM 5669/5406 for my mom as well.

My mom doesn't live in Canada, she just come here to visit me back and forth.

I don't think she needs to fill out those forms if she is not part of the application.
She shouldn't be a "dependent" on the IMM0008 form as well. You may need to attach a letter of explaination since this is a bit unusal.
