Parent - Schedule A Background

Hi Folks,

We are trying to complete Schedule A Background Declaration for my Father and Mother. It asks for Father and Mother information (which is my grand parents). My grand parents are deceased and we have no info. on the date of there birth. So I am putting there as 1900-01-01...

Any input in this situation folks?

Include a separate sheet explaining that you have no documentation for them and so their birth dates are unknown.

I am in a similar situation in that I have no address info for the siblings and they said to write a cover letter explaining.

So do we leave the date of birth blank on the forms and write a cover sheet explaining? Or do we do stars like they ask for our year of birth if its unknown?

Any ideas?

I had a same situation when i applied for my parents and i just put deceased under my grandparents info column
and they accept without any problem.
