OWP process

Hi All,
My husband started his online studies and not yet travel to canada. While applying for OWP for myself and dependent visa for our daughter. There is a form IMM5257e needs to be filled for my daughter. I have a doubts listed below:-

1) what should i fill in the address( visiting location)?
2) how much funds should i mention for our daughter in that form?
