Hi everyone!

My husband and I are just starting the PR process, so my apologies if this question has been asked (or seems stupid).

On Appendix C for the medical appointment, should we be putting HIS UK address or MY Canadian address? We are applying outland from the London, UK Visa office.

We put my address in and so far had no problems. Someone else might have a better understanding but we used the Canadian one after talking to the doctor.
 
Hi

We put our English address down as didn`t have a Canadian one at that time and seemed ok as im now a Permanent Resident!
 
Medical was just last week and the Doctor here put our Canadian address, not the UK one. The Dr. said they do quite a few outland applications so I assume he knows best!
 
Thanks everyone! We ended up going with my Canadian address.

A second question for everyone: When we went to the DMP - she didn't seem to know what to do with Appendix C - I told her that she should take and forward it - however the doctor, nurse, and receptionist ALL claim they have never seen this form before. Hubs and I are feeling a bit uneasy that they didn't know what to do with this form. Any advice here?

In our case, we received a photocopy of Appendix C (along with Copy 1 of the Medical Report Section A) So I assume they forward the original to the Immigration Medical office?!
 
Ugh this DMP didn't seem like they knew what they were doing at all - we had to ASK for Copy of Section A, and pretty much beg them to take Appendix C.

She was on the list so I'm hoping she does everything correctly - we're both just super worried and paranoid at this point.
 
I am in Canada but had my medical done in the states, I took the appendix C with me, when I got there the doctor told me they had their own from the CIC was a triplicate form... anyways, it seems the DMP's in Canada tho they do out-land are obviously set up (form wise atleast) for in-land paperwork, the appendix C is an out-land form and not included in the in-land application kit. There should have been 3 copies of the appendix C, one copy went with the medical done by the doctor, another that goes with the x-ray and the 3rd copy was given to me to mail in with my application, so as long as you have a way for the medical to be matched up with the application all should be fine.

You should have gotten a green sheet. I'd call and tell them this.
