Option C printout for Notice of Assesment

Can someone please help me find this form I need for our application. My common law has not worked since January of 2012 and does not have his tax forms from the US....and I worked in the US up until January of 2012 and I now have my own business in British Columbia and did file my income tax for 2012. Kind of unsure what I am supposed to send to CIC and where to find the form.

I'm pretty sure you still have to call 1-800-959-8281 to order it.

You have to call the Canada Revenue Agency. number 1 800 959-8281. Got the number from the sponsor's document checklist.

My sponsor called this number and talked to an agent who verified the identity first and then asked to a shipping address for the "Option C Printout for the most recent taxation year", which in our case was 2011. Within 10 working days the Option C Printout had arrived.

It is not a form, it is a complete document issued by Revenue Canada by request. All you have to do is attach it to you application.

Sponsor (the one who is Canadian or a Canadian PR) needs to send the printout.
In this country even if you did not earn income, you still are to file taxes. You declare 0.
