OPEN WORK PERMIT APPLICATION FORM QUERIES

Hi friends, i am applying for an Open Work Permit along with Spousal PR (Spouse in Canada). I am currently in Canada on a Temporary Resident Visa. I completed the application form IMM 5710 and validated it. After which there is a message stating 'Validation completed successfully. Please print the entire application and sign and date the last page. This page contains bar codes that facilitate the data entry of your application.' The bar code page has no signature line though.

My question is - do i need to sign and date the bar code page even though there is no signature line? Or is the signature and date on the page before the bar code page on the form enough?

in the IMM5710E form there are below four options, which one you selected while applying for Extension of OWP as OWP is not employer specific
1, Apply for WP with Same Employer
2. Apply for WP for the first time or with new employer
3. Restore my status as worker
4. Get a new temporary resident permit

thanks for the update ad while applying can i just add the offer letter from my husband's employer and 1-2 payslips and update the remaining payslips later?

thanks for replying...and so i need to apply 30 days before or can apply later also like 2-3 weeks before the expiry of WP?
once applied can i continue to work?
