When registering my baby’s birth, it asks for my current last name and my last name at birth. Does anyone know which one gets put on the “birth certificate with parental information”?

I think it should be maiden (name at birth) for genealogical purposes but I do not know for certain.

Thanks for your reply. I actually did call and got different answers both times lol. I thought maybe someone here who recently registered a birth could let me know.
