Old address on Medical request forms. Will this be a problem ?

Hi All,

I received medical requests for myself and my wife and found old home/mailing address on the medical forms. I have already updated my new address few months online and it is showing correct address. I have sent an email to CPP Ottawa requesting to update my records with new email address.

Now, can i proceed for medicals with the forms (which has old address) ? Or do i need to ask CIC to send new forms with updated address ?

Could someone please provide your opinion.

Proceed with the form (with your old address on it) . Doctor can change your address for medical.

I also had a similar issue.
you just proceed with the medical and send an email to ottpilot explaining your case after the medical.
They can take care of it.
