Panel physicians do not take appointments for exams now unless you've gotten a request. That was part of the changes made to the spousal sponsorship kit back in December. The option of doing them upfront is no longer available.

Anywhere from a few days to a couple of months. It's hard to say, however maybe it's worth checking out the thread for your VO and see how long others have took :)

Nah, it's not mandatory. You can even use your previous name and change it later on. But no it's not a requirement.

Finally got a message from Manila Visa office. They are requesting the following documents which I submitted already

1. Medical Report - I had an upfront medical last November 21, 2016. Enclosed the e-medical report as instructed by the clinic.
2. Philippine Statistic Authority Certificate of No Marriage - submitted the original. The good thing is I scanned so I have still a copy.
3. Written explanation regarding my relationship to the mother of my son - i don't know what should be the content of this letter. Hope you guys can help me. Does anyone has a circumstances like this?

How do you ask for "notes?"

Also, I am Canadian living in US with my spouse who I am sponsoring, but now I'm thinking of moving back to Canada in advance. BUT, if my spouse decides to come with me, and then file an open work permit, will our application change? Or do we need to start over again as INLAND? Is this a dumb idea? Obviously we just don't want to be separated.

I think our plan is that I am going to move up in May. If my spouse is approved before then, beautiful, if not, then at least I will be up there working with an apartment which will look even better on our application. It will be an easier transition for my spouse as well as she will be the only one looking for work instead of both of us if we moved up at the same time.

And as for the notes, what exactly do they tell you, just the status? what does it mean for the deadline date, that the review is over and they move onto the back ground check? We applied in November, so its already been 4 months for us and we are in the US.

Truth be told, unless things slow down drastically at Ottawa, I wouldn't be surprised if your background check starts before you even get your notes, and you could have DM before then. We're at 5 months and change and our COPR arrives tomorrow.

Unless there's something highly unusual about your case, I can't imagine you wouldn't already be approved before May.

5 months and change? that's great news! what is DM? I'm not good with acronyms. We're planning on responding tomorrow that I'll move back to Canada May 1st whether or not my spouse is approved and the less time we're apart, the better. Hopefully it'll be quick! 8)
