Super Visa application for my mother. Her Indian passport has no surname — her full name is " XXXX YYYY" appears under Given Name only.
On the application I entered her full name in the Family Name / Last Name field and left Given Name blank (the form wouldn't validate otherwise).
My question: on the representative form (IMM 5476), should I enter her name the exact same way — full name in Last Name, Given Name blank — to match the application? Or does the rep form follow a different rule, since it says to enter the name as in the passport?
