We have been checking online for the medical instructions and finally found out that they said that they sent them in August but we have heard nothing.

We have been having issues linking the application online, but we are sure that they sent an email. What can we do now? How can we get the medical instructions sent again?

You could try filling out a webform and requesting it, or you could try calling and speaking to a representative. I've heard though that you'll be on hold for literal hours before you get through to anyone, and that the phone representatives frequently give incorrect information... so you may want to stick with the webform even though it might be a week or so before they reply.

Also I don't know if you've seen this already, but this thread below has good tips on linking an application to an online account and goes into a lot of detail. Apparently it's not uncommon for it to take several tries for it to work.
