I am about to send my completed application. Just a couple of quick questions regarding birth certificate:
I don't have birth certificate. However I can provide Class X mark sheet, PAN Card & passport copy as an evidence for DOB. Do I need to submit an affidavit mentioning the reason that why I am unable to provide one or just declare my DOB in sworn affidavit. Can any one share the format of such affidavit?
Secondly my daughter's name is not mentioned on birth certificate. It just says a baby was born to (parent's name) on so & so date. However she has a valid passport. Do I need to submit an affidavit for my daughter's DOB as well?
This is a most common situation I am sure most of you would have come across. Help Appreciated.
