No Biodata form, HELP please!!!

Hello guys,

I was asked to send passport, 2 photos and BIODATA form to CIC via regular mail last week. But CIC did not attach the BIODATA form in their email as it should be. I emailed CIC back for it but get no reply.

Would anybody please email me a blank form to Beijing780 @ gmail.com ? (Please DELETE the blank before and after @)

My work permit will expire soon so this issue drives me crazy....
