New passport-urgent

Hello guys i want to apply for a new passport however there is a section where they request parents surname at birth .
It doesn’t specify whether it is mother of father surname.
Coule you clarify me
Also there is a section regarding employment history
I put unemployed in employer name and for the rest such as phone number,address,date,field etc i put N/A
Is that ok?

The form that I have says "Mothers surname at birth" . Is there a change?

Thanks a lot much appreciated. I was confused whether i put the surname of my father or my mother
