This might sound stupid pero question po sa mga nag request na ng gcms notes. Kelangan pa ba tlga ng consent for an access to information and personal information request na signed by my husband in the Philippines? Ano po ginawa nyo? Inemail nyo nlng sa knya tas he signed then iniscan at sinend back to you? o pwede bang wala na lang syang pirma?

Kailangan ng signature. He can download the form from CIC website, sign it, scan/take a picture with it and send it to you. Actually for our GCMS notes request, nagkamali ako ng fill up ng form. They emailed my spouse to send in the new corrected form before they released the notes to him.

Hi were planning to requeat for GCMS notes din...and yes kelangan talaga ng pirma niyo both sa consent...and the only way is pirmahan niya scan send sayo print mo sign and scan.its better to be safe than sorry...

You will fill up your info (sponsored spouse) on section A of the form and the Sponsor's info on section C. They emailed my spouse to send in the corrected consent form before they released the GCMS notes to him.

Hindi na but they send the email to the person who requests the GCMS notes (your sponsor). So when my spouse got the email, I printed the form, signed it and sent it to him and he then forwarded it to ATIP/CIC.

you're welcome! Hindi pa, I only got the Appendix A request + immigration photos email last September 21, 2015. No PPR, I think because I'm currently living in a different country (US) so maybe they thought it's no use to hold on to my passport for months when I might need it here. On my GCMS notes, our application's "due date" is on December 15, 2015. I asked on a different thread and it means that's when they'll next open my file so I'm not expecting any update until then
