These medical instructions are being isued since your aplication has ben procesed to the stage
wher medical examination results are now required. Please read these instructions carefuly.
IMPORTANT: You are required to undergo the medical examinations within 30 days of the date of this
leter. Failure to do so could result in the refusal of your aplication.
The medical examination must be performed by adoctor fom the CIC list of Panel Physicans.
You canot chose your family doctor ifhis or her name is not on this list. Se the list of Panel Physicans to
find adoctor in your area t:
Once you identify apanel physican in your area, bok an apointment for your medical examination.
When you schedule your apointment, please nsure that arangements are made for your chest x-ray
and laboratory test as wel. Once your medical has ben completd the doctor wil forward the medical
results to CIC.
Any cost related to the medical examination are your esponsiblity and are payable at he time of the
examination. Should your aplication be refused folowing the completion of your medical examination
these cost wil not be refunded.
Please bring the folowing to your apointment:
- The atached Medical Report form (IM1017E)
- Proper identifcation (prof of identiy must include at least one government-isuedocument
with photgraph and signature, such as a pasport or national identiy card)
- Eye glases or contact lenses, ifworn
- Any medical reports or test results that you may have as aresult of previous or existing
medical conditons
- Fourecent photgraphs (phots are not required if the panel physican is listed as “eMedical
enabled” on the CIC website listed above)
Note: It is importanthat you inform the panel physican of any previous or existing medical conditons.
Failure to do so wil delay the procesing of your medical examination.

Where should he send results , is there a email of Delhi embassy that he should send results

I received a mail 20 mins ago as

This refers to your application for admission to Canada.

In order for us to continue the processing of your application, we require the following:

A.Photographs of your Hindu marriage showing essential and actual marriage ceremonies.

B.Period of staying together after the marriage

C.You have not provided Page one of your marriage certificate, therefore, please forward the same

D. Please indicate exact relationship of your brother With the sponsor

It appears that itzpreethi and nabaaf are in the same cohort as me.

My wife also received a similar email to the above. It looks like its a document request about more pictures of the marriage and information re: co-habitation. I guess its because my wife came to visit me in Canada in Oct-Dec 2015. She plans to send the email back to them with the relevant information by May 31, 2016.

One question: They didnt ask about remeds. I am pretty sure that my wife's upfront medicals have now expired. Is it reasonable to assume that she will receive a re-med request soon?

It seems to be full medical as no specific tests r mention....
Email received at 3:49pm (India Time) 30th May 2016....initial medical expired on 20th April 2016
22nd June 2015 applicant
Booked my appointment fr tommorrow morning....
