You can download the form from here http://www.cic.gc.ca/english/pdf/kits/forms/imm5744e.pdf and PART A need to be filled by Principal Applicant and fill Part C for person who is going to receive the notes.You can submit the request here https://atip-aiprp.apps.gc.ca/atip/welcome.do?lang=en and it cost $5.

oh cool my wife is close to edmonton.....can you please inbox the mp's details....the mp she went refused to do enquiry before 19 months. is it necessary to be from the same area to do mp enquiry?

For my MP enquiry, I used two different MPs.
One from my living area, and one from my working area

You do have to be leaving in that particular MP's jurisdiction in order for them to do any inquiry.

What was the processing time when you applied?May be you can make a case stating that the processing time changed few times since you applied.This is what i am going to do if don't here anything by Friday next week.And processing time changed twice on me since i applied for my wife.

As far as my understanding is you don't need to send anything.The doctor will upload your medical to CIC.But this is the email id DELHI-IMMIGRATION@international.gc.ca i have used in the past but please count on this.

hey you should try the mp of garry if your spouse is in edmonton....that mp seems to be very cooperative. my wife is not in Edmonton.
