Need some help on Statutory Questions

Hi Friends,

I read lots of discussions on how to answer Statutory questions on EE profile. However, I didn't remember I ever completed that part! I hired a certified representative to complete the form for me and I didn't find the Statutory question part on the screenshots he sent to me. Did all of you come across those questions?
