My wife the ( applicant ) uses my surname to get her police clearance and medical,but her passport all the documents she I wrote her maiden name. Is that ok?

I am pretty sure that when your wife goes for her medical and police check she will be required to show valid identification (such as passport). Theyl will be done according to the name on the documentation provided. So if she shows her passport (and it is in her maiden name) then the police check and medical will be done using her maiden name.
 
The name on her passport is the most important one to CIC. It helps to do everything with the same name for consistency. You can put her married name where it asks for aliases.
 
When she got nbi ( police clearance in the Philippines) she used my surname so it's ok to send the all documents she wrote maiden name.she Foreign worker.I just to ask if she need to get a police clearance here in Canada? Because she live here more than 4 years?.

Yes, it will be fine, and not uncommon in these types of applications. On her immigration application she will need to use the name on her passport and not her married name. The name on her passport/identification document is the one that CIC will care about. She can list using her married name as an 'alias' name as suggested by the other posters responding here.
