My grandmother came to Canada to visit us ten month ago, she got very sick and was not able to fly back home ( she has US Green Card) we applied to immigration office to extend her visa; now we got a letter from them that her visa is denied and she has to leave immediately. She is very old and we don't know what to do, can we appeal ?
 
You can apply for the restoration of status within the 90 days of your application is rejected. Even if they reject your restoration of status application, you literally "buy" about 3 months of time for her stay.

Can you do that knowing fully well that the extension has already been denied?

Sure you can try, it only costs money, but probably another refusal.

If you went past your allowed stay (without having requested for an extension, because if you have submitted an extension request you are on implied status) but within 90 days of the expiry of your valid stay, you can submit an application for restoration of status- the same form you use when you request extension of stay except you tick restoration (D).
