I am sponsoring my wife. It is an outland application.

My questions are:

1) The 5406 Form needs to be filled out by my wife: right?

2) For section A: APPLICANT is my wife and the Spouse should be me: right?

3) Section A asks to write name also in native language. How do we do that? After typing name in English, also write their name in native language (Bengali)? OR better to use a separate sheet of paper?

4) Who signs Section A?

5) Who Signs Section B? (We don't have any children)

6) Who signs Section C? (only my wife's brother/sister names goes here right?)

7) Who signs Section D?

Of all the Forms, I found this one most difficult to understand.

Correct.

If there's room, try putting it on the same sheet.

4) Who signs Section A?

7) Who signs Section D?

For ALL of these, your WIFE signs. You only sign second B if you have no children. Section C is your sibling-in-laws names and no one signs. Hope that helped!

Also, note that you only sign the certification in section A to certify you aren't married (so it wouldn't apply in your wife's case :) ).

Also, write names in all sections (not just A) in English and the native language (if it fits on the form, in the same box; otherwise in a separate sheet of paper)

They use this form to define the applicants family both to assess admissibility now and also for any potential future applications.
 

Parker 24, thanks. But, I am still a little confused. Do I sign section B (neither of us have any children)? Signature on Section C is to be left Blank?

No, your wife does. There's no place to sign on Section C ;)

Goodman,

Your wife (the one who you are sponsoring should fille out this form)
Your wife should sign section B and D but section C has no signature line just fill out the information
 
Also make sure to to put N/A in section B where it ask for children
 
Even if I am just bringing my wife here I have to fill out this application form. A friend told me that I dont need this form because the application is only for my wife, and this is for "additional" family members, and since there are no additional members applying then this form does not need to be filled out. Is this true? or must this be filled out regardless?

Your wife has to fill it out to disclose her family members, regardless of whether they are currently being sponsored or not and whether they will be accompanying or not.

Think of it this way - it discloses who she can ever sponsor in the future ;)
