Need help for Police Certificate

Hello guys,

I am in process of gathering documents for spouse sponsorship application outland applicant. When I went through cic guide 3999, it says that we need to include the completed 'Request for Police Certificates/Clearances and Authorization for Release of Information' form while applying for police certificate. Does it mean that I should fill up this form only to obtain original certificate, isn't it? In case for application package, I should only include the original certificate not the form itself, am I right? Thank you

Which country PCC are you trying to obtain ? Depending on that you may or may not have to even include this Request form.

You should submit only the original PCC's to CIC. Not required to submit this form. It is just a request form.
