Need application form Link to apply Visitor permit to Spouse using my BOWP

HI Guys,

Currently myself and my wife are staying in Canada . My wife is in visitor permit ( dependent to me) and it will expiry in 31st Jul 2015. I received my BOWP now, I would like to extend my spouse Visitor record by using my BOWP. Please provide forms link to extend visitor record permit.

You should've have renewed her visitor record when you renewed your BOWP, that how I did it
