need answers in applying Pr card

It is my first time to apply for the pr.
I came and study here from 2011_2013.and work from 2013_now.never left canada.I download all the documents from the website.The IMM5455, supplementary identification form makes me feel confused. Shall I print it out and THEN fullfill it by hand writing or complete it on computer and print it out?
And the last line" date of permanent residence", which date I have to put in? The first day I arrived Canada or the day I apply for the pr card?
Pls do help me this it is my first time appy for Pr .now I am with open work permit , it will expire soon.

Your post is a bit confusing. Are you already a permanent resident? Or are you still here on a work permit and are applying for PR now?

In that case you don't qualify to apply for a PR card. You first have to apply for permanent residency. To apply for permanent residency, you have to select an immigration stream (i.e. which immigration program you are going to apply through). You can find information about Canada's various immigration programs here: http://www.cic.gc.ca/english/immigrate/apply.asp You will have to research the options to determine which program you qualify to apply under and what it takes to apply.
