Need advice on - Employer letter format

I have received invitation in recent draw , However i have few doubts.
1. Should I include NOC code in reference letter ?
2. What is the correct format for the reference letter?
3. I worked with an employer for pay of 31$ /hr for 11 months (2015 feb to dec) and pay was reduced to 22$/hr for the next 2 months . Will there be a problem regarding this pay reduction? ( worked as a SOFTWARE TESTER)
4. After this I left the job and joined in another company after 3 months as Computer network technician with pay of 11.40$/hr .

Will there be a issue in background check ?

What is that for the 4th question? I meant will there be a issue since I was working for minimum pay as the contract from the previous company expired.
