Our PR application for the Live-in Caregiver Program is being processed and they asked for some additional documentation for my spouses's child, who still lives in the Philippines. The child's birth certificate is late registered and he is 14 years old. Can anyone confirm if I need his Birth Certificate and Baptismal certificate and school records for dependant children 6 and over and Notarized Joint affidavit of two witnesses with clear photocopy of a valid and legible identity document with signature and Affidavit of delayed registration? If the NSO Birth Certificate was Late Registered we require that you submit the following:

  • Baptismal certificate and medical record for dependant children below the age of 5
  • Baptismal certificate and school records for dependant children 6 and over
  • Voter’s affidavit for applicants that are 18 years old and over.
  • Marriage contracts of parents, if they are married
  • Notarized Joint affidavit of two witnesses with clear photocopy of a valid and legible identity document with signature
  • Affidavit of delayed registration

Note - If a father is listed on the Birth Certificate, please. provide details of your past and
present relationship with your dependent’s father as he is listed on the birth certificate.
